What We Need From You:

* Very clear photo or photos of your pet's head from the front view
* The photo should be taken at your pet's eye level
* A completed Order Form with your Pet's Name and details
* A 50% deposit via personal check, credit card payment through PayPal or call Anne with your VISA/MASTERCARD information (773.772.1085). 
* A clear understanding that Anne retains all copyright and reproduction rights on the image she creates of your pet.
